dc.description	This Model of Care for COPD has been developed by the National Clinical Programme for COPD in order to define the way health services for people with COPD are delivered. The document outlines the best practice integrated care and services for a person with, or at risk of developing, COPD as they progress through the stages of their condition. Specifically, the key aims of the Model are to: • prevent or delay the onset of COPD • improve the delivery of care to people with COPD across all levels of care • save the lives of people with COPD Through the implementation of this Model of Care, the Irish health service will be ensuring that the right care is delivered to people with COPD at the right time and in the right place.	en_US
