dc.description | The Galway University Hospital Pathfinder team launched on 29/05/2023. The team is uniquely integrated within Saolta hospital group’s IT and Electronic Patient Record (EPR) systems. Prior to launch, the team worked collaboratively and proactively with IT colleagues in order to ensure access to hospital EPR systems, to create ‘Pathfinder’ documentation proformas and to develop the Pathfinder documentation standard operation procedure. All members of the Pathfinder team have access to relevant and appropriate EPRs, including scheduled and unscheduled care information and IT platforms. EPR access has allowed more accurate and up to date clinical information gathering as part of the Galway Pathfinder assessment, treatment and patient management. | en_US |
